  • CRAFFT 2.0 (Car, Relax, Alone, Forget, Friends, Trouble) developed by Boston Children’s Hospital in collaboration with the American Academy of Pediatrics is used to screen adolescents (ages 12-18).
  • Brief Opioid Overdose Knowledge (BOOK) developed by researchers at Johns Hopkins University is a twelve-question screen to assess opioid knowledge, opioid overdose knowledge, and opioid overdose response knowledge.
  • The Patient Health Questionnaire 9 (PHQ9) is a 9-question instrument given to patients to screen for the presence and severity of depression.
  • The Generalized Anxiety Disorder 7 (GAD7) is a screening tool to measure anxiety.
